What is being claimed?
Wegovy plus a secret weapon. Amylin is the missing satiety hormone. Better than semaglutide alone. The next default after or instead of tirzepatide / retatrutide.
Patients who already know GLP-1s often assume they already understand the conversation. The new piece is amylin, not another version of semaglutide.

Fixed-dose cagrilintide (long-acting amylin analogue) + semaglutide (GLP-1 RA), once weekly, with lifestyle intervention in the Phase 3 programs. Two pathways. One injection.
REDEFINE 1 (no diabetes)
Large Phase 3a trial published in NEJM (2025). Arms include CagriSema vs semaglutide alone vs cagrilintide alone vs placebo. That active-control design lets you ask whether amylin adds anything beyond semaglutide alone, not only whether the combo beats placebo.
Mean weight reductions with CagriSema are large vs placebo. Company and symposium materials often cite about 20.4% under a treatment-policy estimand and higher figures under trial-product estimands. Prefer estimand-aware language.
REDEFINE 2 (type 2 diabetes + overweight/obesity)
Published NEJM (2025). Mean weight change about −13.7% vs −3.4% placebo at 68 weeks. Glycemic responses strong in reported analyses. GI events common.
REDEFINE 4 (vs tirzepatide)
Company topline February 2026. Open-label Phase 3 (~809 adults, 84 weeks): CagriSema 2.4/2.4 mg vs tirzepatide 15 mg. Absolute weight loss with CagriSema was large (~23% efficacy estimand; ~20.2% treatment-regimen), but the primary endpoint of non-inferiority versus tirzepatide was not met (tirzepatide higher). The December 2025 NDA rested on REDEFINE 1 and 2, not this readout.
Regulatory
NDA submitted December 2025 for weight management. U.S. decision often framed for late 2026 in company guidance; no public PDUFA confirmed here. No approved CagriSema product yet.

Why should an RD care?
Amylin literacy. Short and non-mystical: amylin is a hormone made with insulin that helps regulate satiety and food intake. Cagrilintide is a long-acting version. CagriSema pairs it with semaglutide.
Combo GI realism. Two GI-active agents. Expect familiar nausea/fullness/constipation themes, possibly louder during escalation.
Comparison literacy. CagriSema isn’t retatrutide. Different company, different architecture (combo vs triple mono-agonist), different evidence package. REDEFINE 1 asks whether amylin adds anything beyond semaglutide. REDEFINE 4 asks how the combo sits next to tirzepatide (non-inferiority not met). Keep those questions separate.
While waiting. People already on approved semaglutide or tirzepatide still need protein, resistance training, and maintenance thinking. A filed NDA does not replace nutrition care on approved therapy.
Usable line: “CagriSema is a weekly shot that combines semaglutide with an amylin medicine. Phase 3 trials in the NEJM show meaningful weight loss, including comparisons that help ask whether amylin adds something beyond semaglutide alone. A later head-to-head against tirzepatide did not meet non-inferiority on weight loss. It isn’t FDA-approved yet. Knowing Wegovy helps, but amylin is the new piece you’d need to understand.”

CagriSema is an investigational once-weekly fixed-dose combination of cagrilintide (amylin analogue) and semaglutide (GLP-1 RA), with published Phase 3 NEJM packages in obesity without diabetes (REDEFINE 1, including active controls) and in type 2 diabetes with overweight/obesity (REDEFINE 2). An NDA for weight management was filed in December 2025; the product isn’t approved yet. REDEFINE 4 showed large absolute weight loss but did not meet non-inferiority versus tirzepatide. The distinctive teaching is amylin entering metabolic counseling beside a familiar GLP-1, not another informal “GLP-3” story. A usable stance today is curious about the combo’s published efficacy, precise about architecture, honest about the tirzepatide H2H, realistic about GI burden, and unwilling to pause today’s counseling for a pending label.
