What is being claimed?

The Ozempic / Wegovy pill. Same results as the shots. Adherence solved because there is no needle. Milder side effects because it is “just a pill.” First and only oral GLP-1.

People who already know injectable GLP-1s often assume the delivery method is the only thing that changed. Chemistry, ritual, magnitude, and label boundaries all moved.

Chemistry and ritual

Orforglipron is a non-peptide small-molecule GLP-1 receptor agonist. Foundayo is a once-daily tablet.

PI-aligned administration: once daily, with or without food; swallow whole (do not break, crush, or chew); one tablet per day. That is the regulatory object. Company materials also contrast it with oral semaglutide’s fasting and limited-water requirements; keep that comparative, not as invented label text.

Labeled strengths: 0.8, 2.5, 5.5, 9, 14.5, and 17.2 mg. Escalation: start 0.8 mg → after ≥30 days, 2.5 mg → after ≥30 days, 5.5 mg → then may step to 9, 14.5, and 17.2 mg after ≥30 days at each level, by response and tolerability. Maximum 17.2 mg once daily.

Capsule → tablet map (older ATTAIN papers often use capsule mg): 1→0.8, 3→2.5, 6→5.5, 12→9, 24→14.5, 36→17.2. So ATTAIN arms reported as 6 / 12 / 36 mg capsules correspond to 5.5 / 9 / 17.2 mg tablets. Prefer labeled tablet names after that explanation.

CYP3A4 (PI): maximum 9 mg once daily with a strong CYP3A4 inhibitor; avoid strong CYP3A4 inhibitors that also inhibit OATP1B; avoid strong CYP3A4 inducers; with moderate inducers, monitor effectiveness and escalate as needed.

ATTAIN-1 (no diabetes)

Published in NEJM (2025). About 3,100 adults. At 72 weeks, treatment-regimen mean weight reductions on the order of 7–11% across studied doses versus about 2% placebo, largest at the top dose (~11.2%). GI events class-familiar. Discontinuations for adverse events higher than placebo.

ATTAIN-2 (type 2 diabetes + overweight/obesity)

Published in The Lancet (2025). Mean weight reductions smaller than ATTAIN-1 at matched dose levels (top dose on the order of ~9.6% vs ~2.5% placebo under the treatment-regimen estimand), with glycemic improvement.

ACHIEVE and oral comparison

Diabetes Phase 3 includes a head-to-head versus oral semaglutide (ACHIEVE-3, The Lancet): orforglipron showed greater A1c and weight reductions in reported analyses, with more GI burden and discontinuations in that comparison. Useful for oral-versus-oral tradeoffs. Not a substitute for a diabetes label.

Regulatory

Foundayo approved April 1, 2026 for chronic weight management with diet and activity. Diabetes indication pending as of July 2026.

Why should an RD care?

Object literacy. Foundayo is orforglipron: oral non-peptide GLP-1. Not liquid Ozempic in a capsule, not compounded “oral sema,” not interchangeable with Rybelsus by brand folklore.

Ritual literacy. Prior failure on oral semaglutide because of empty-stomach / limited-water rules may not transfer. Expecting zero GI because “pills are gentle” will. Foundayo’s PI object is simpler: once daily, with or without food.

Expectation literacy. ATTAIN averages are meaningful and usually quieter than dual-agonist injection marketing. Match goals to the product, not a headline from a different molecule.

Label boundary. Weight-management conversations can reference an approved product. Diabetes-only use still waits on indication and PI.

Same living-well work. Protein, resistance training, GI pacing, shame detox, maintenance thinking. Delivery form does not retire nutrition care.

Usable line: “Foundayo is a daily GLP-1 tablet. It’s a small molecule, not a peptide shot. The label says take it once a day with or without food; it doesn’t use the fasting and limited-water rules that oral semaglutide needs. Phase 3 trials showed meaningful weight loss, usually less on average than the strongest injectable dual agonists. It’s approved for weight management; a diabetes indication isn’t approved yet.”

Orforglipron (Foundayo) is a newly labeled once-daily oral non-peptide GLP-1 receptor agonist for chronic weight management, with published Phase 3 ATTAIN packages and an active ACHIEVE diabetes program that still lacks a U.S. diabetes indication. What the landscape makes hard to ignore is not a slogan about ending injections. It is a new counseling object: different chemistry from peptide GLP-1s, different daily ritual from oral semaglutide, real but generally quieter average weight-loss magnitudes than the loudest injectables, and living practice that has barely started. An intelligent stance today is precise about the object, honest about expectations, careful about indication boundaries, and unwilling to treat “no needle” as a complete clinical argument.
