Exercise
This page is about structured exercise for weight, muscle, and fitness. Average weight change is modest, often not more than about 2 to 3 kg with aerobic training. That modest average is not a reason to ignore exercise, and it is not a rival to medicines. Rankings here are not rankings of who prevents heart attacks.

Bottom line
Exercise stays on the map because different modes do different jobs. Aerobic training is preferentially advised for modest weight and fat loss averaging not more than about 2 to 3 kg. Resistance training is advised to help keep muscle during weight loss. Any mode can be used for fitness and insulin sensitivity. That modest average is not a reason to ignore exercise, and it is not a ranking of who prevents heart attacks or who matches a shot.
What it is
Planned training repeated over weeks: aerobic exercise, resistance training, combined programs, interval training, and related modes. Different modes have different jobs. It is not a claim that any one viral workout wins on heart outcomes or matches a shot on kilograms.
What taking part usually involves
Participation means repeating a training plan over weeks. Aerobic training at moderate intensity is the mode preferentially advised for weight and fat. Resistance training at moderate-to-high intensity is advised to help keep muscle while you lose weight. Interval training in this package comes after a heart-risk check and under supervision. This map does not crown a single best brand of exercise, and it does not retire walking.
What it is commonly confused with
“If the average is only a few kilograms, exercise doesn't matter.” “Exercise should be judged against tirzepatide.” “Interval training is the only real exercise.” “Fitness gains mean heart-attack prevention.” “Joining a weight-management program is the same as choosing a training mode.”
What the evidence shows
An expert synthesis of exercise reviews advises aerobic training for modest loss of body weight, total fat, visceral fat, and liver fat, with expected average weight loss not more than about 2 to 3 kg. Resistance training is advised to help preserve lean mass during weight loss. That is a different job from kilogram loss. Any studied mode, including interval training after assessment and with supervision, can be advised for insulin sensitivity and cardiorespiratory fitness.
Those three jobs are why a small average weight change is not irrelevance. Medicines that lose more weight are answering a different question.
Mode ranking here is about composition and fitness. It is not a ranking of heart-attack prevention. Walking is not obsolete because interval training exists. Unsupervised interval training does not inherit supervised interval training.
Joining a structured weight-management program is a different decision. It lives on the programs page.
Who was studied, expectations, and limits
Population and context studied: adults with overweight or obesity in exercise reviews measuring weight, fat, lean mass, fitness, blood pressure, and insulin sensitivity.
That is the research setting, not a personal clearance for every workout.
Realistic expectation: aerobic training can produce modest fat and weight change; resistance training can help keep muscle during weight loss; several modes can improve fitness. Limits: modest average weight is not event prevention; not a rival to medicines on kilograms; dedicated long-term maintenance trials are thin; not a program page.
